Here at Allen Haven Therapies, we are happy to accept and file insurance claims for the following:

Personal Injury Plans: Auto & Liability
Worker's Compensation

Referrals may be obtained from a Medical Doctor, Doctor of Osteopathy, OB-GYN, Nurse Practitioner or Chiropractor.

Patient responsible for all fees. Medically prescribed therapy may or may not be covered by insurance.
Fees: $30.00 per Unit
